Evolve Therapy Services is hiring a Physical Therapist for an in-house Skilled Nursing Facility in Rochester, MN. The role supports planning, directing and delivering PT services ensuring quality resident care in compliance with federal/state standards.

Full-time or PRN options with emphasis on documentation, evaluation, and treatment planning. Providers must hold a BS in Physical Therapy and an active state license, with ongoing CEU and professional development required to maintain licensure.

Formación

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsabilidades

• Provide physical therapy services to residents according to established standards and procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation and follow-up of resident therapy programs.
• Conduct screenings of residents to determine need for intervention/treatment.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop treatment plans with short and long-term goals and methods to achieve them.
• Interpret and communicate evaluation findings and recommendations to residents and the interdisciplinary team.
• Complete CMS-compliant and medically necessary documentation in NetHealth EMR.
• Perform other duties as assigned to support organization goals.

Educación

Bachelor of Science in Physical Therapy

Active Physical Therapy license in states of practice

Ongoing continuing education and professional development
