Evolve Therapy Services in Minneapolis, MN seeks a Physical Therapist for an in-house Skilled Nursing Facility. This PRN role supports planning, implementing, and directing physical therapy services to ensure high-quality resident care.

You will perform evaluations, develop treatment plans, document in NetHealth EMR, and collaborate with the interdisciplinary team to achieve patient goals. Requires current PT license and BS in Physical Therapy; CE activities encouraged.

Formación

• Bachelor of Science in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsabilidades

• Provide physical therapy services to residents following standards and procedures.
• Adhere to documentation and billing procedures.
• Evaluate residents to develop treatment plans with short- and long-term goals.
• Communicate findings and recommendations to residents and the interdisciplinary team.
• Screen residents to determine need for intervention and treatment, and perform timely screenings.
• Develop treatment plans and interpret evaluation findings for residents and families.
• Complete compliant CMS documentation in NetHealth EMR systems.
