Job summary

Evolve Therapy Services is seeking a Physical Therapy Assistant for an in-house skilled nursing facility in Lawton, OK. You will provide therapy under the supervision of a Physical Therapist and help implement treatment plans for residents.

The role includes conducting screenings, contributing to goal-setting, and delivering individual and group treatments, with CMS-compliant documentation. This is a Full-Time position supporting resident care needs.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.
• CMS expectations documentation in NetHealth EMR.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in evaluations and treatment planning under the direction of the Physical Therapist.
• Assist in establishing short- and long-term goals and treatment plans.
• Conduct individual and group treatment as outlined in the plan.
• Train residents and appropriate staff in the use of equipment.
• Participate in resident, family, and staff education.
• Maintain documentation per CMS expectations in NetHealth EMR.

Education

Associate's degree in physical therapy

Active Physical Therapy Assistant license in state(s) of practice

Continuing education to maintain licensure
