Job summary

Evolve Therapy Services is seeking a Licensed Physical Therapy Assistant to work in an in-house Skilled Nursing Facility in Oklahoma City. You will provide therapy services under the supervision of a Physical Therapist, conduct screenings, and assist in developing and implementing treatment plans.

The position offers Full Time, Part Time, or PRN options. Applicants must hold an active PTA license and maintain ongoing professional development.

Qualifications

• Associate degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Assume responsibility for ongoing continuing education and professional development.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in evaluation and treatment planning with the Physical Therapist.
• Identify goals and treatment plans with the Physical Therapist.
• Conduct individual and group treatments as outlined in the treatment plan.
• Monitor residents’ responses and report modifications as needed.
• Assist in scheduling and coordinating resident therapy with the team.
• Communicate with residents, families and staff regarding therapy goals and progress.
• Ensure documentation aligns with CMS expectations in NetHealth EMR systems.
