Job summary

Powerback Rehabilitation is seeking a Speech Language Pathologist to provide in-home outpatient services for older adults. You will evaluate and treat in patients' homes or in assisted living settings, promoting functional communication and safe swallowing.

The role emphasizes flexible scheduling and collaboration with the rehabilitation team. You must hold a valid state license (or eligibility) and a valid driver's license.

Qualifications

• Must have a valid Speech Language Pathologist license in the state(s) of practice, or proof of license eligibility.
• Must have a valid driver’s license or reliable transportation to and from patients' residences.

Responsibilities

• Provide outpatient rehabilitation services in-home for the older adult population.
• Supervise CFYs, temporary licensees, students and support personnel per licensure requirements.
• Ensure timely clinical and billing documentation and participate in care conferences.
• Develop and adapt speech-language treatment plans with patients and caregivers.
• Coordinate care in home and community with other rehab team members.
