Responsibilities

• Provide outpatient rehabilitation services in an in-home setting for older adults.
• Work in patients' rooms in assisted living facilities or independent living facilities.
• Organize and provide speech-language services for residents in the facility.
• Direct patient participation in selected tasks to restore, reinforce, and enhance performance.
• Facilitate learning of skills essential for communication and swallowing to remediate deficits.
• Maintain safety and enforce fire and safety procedures within scope of practice.
• Supervise CFYs, temporary licensees, students, and support personnel as applicable per state requirements.
• Adhere to department and patient treatment schedules; complete timely clinical and billing documentation.
• Attend and contribute to patient care meetings and staffing conferences.
• Make referrals to other disciplines to address comprehensive patient needs.
• Consult with rehabilitation team and physician to select appropriate therapy regimens.
• Adapt program and plan of care to individual patient needs.
• Coordinate with interdisciplinary team in the facility and referring physician.
• Promote a safe environment and prioritize patient service quality.
