Job summary

Beyond Home Health Care in New Jersey seeks a licensed Speech-Language Pathologist to deliver in-home and community-based speech, language, and swallowing services to adults with developmental disabilities, aligned with NJ DDD guidelines and person-centered planning.

The role involves evaluations, treatment planning, one-on-one therapy, collaboration with supports coordinators and families, caregiver training, and meticulous documentation and billing.

Qualifications

• Current New Jersey license as a Speech-Language Pathologist (NJ ASHA-approved).
• ASHA CCC-SLP certification required; CFs may be considered with supervision.
• Master's degree in Speech-Language Pathology from an accredited program.
• CPR/BLS certification and clean background checks are required.

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans aligned with ISP and goals.
• Deliver one-on-one therapy in the participant's home and community settings.
• Document services per DDD standards: session notes, progress notes, outcome measures.
• Collaborate with Support Coordinators, families, caregivers and the interdisciplinary team.
• Provide caregiver training and home programs to reinforce goals.
• Adhere to DDD policies, laws and HIPAA confidentiality.
• Maintain accurate billing records consistent with DDD rates and authorizations.
• Complete required DDD trainings (pre-service, Danielle's Law, etc.).
