Job summary

Beyond Home Health Care seeks a licensed Speech-Language Pathologist to provide in-home and community-based services for adults with developmental disabilities across New Jersey.

The role involves evaluations, individualized treatment, caregiver training, and collaboration with an interdisciplinary team while complying with DDD policies and HIPAA. Per diem/contract schedule offers flexible hours and a supportive environment.

Qualifications

• Current New Jersey license as a Speech-Language Pathologist.
• ASHA CCC-SLP certification required; CFs considered with appropriate supervision.
• Master's degree in Speech-Language Pathology from an accredited program.
• Minimum of one year of clinical experience preferred; CFs and new graduates considered.
• Current CPR/BLS certification.
• Clean criminal background check and fingerprinting per NJ DDD requirements.
• Reliable transportation, valid NJ driver's license, and current auto insurance.
• Ability to pass DDD background checks including CRoff.

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans aligned with ISP and person-centered goals.
• Deliver one-on-one therapy services in the participant's home and community settings.
• Document services in compliance with DDD documentation standards and submit within required timeframes.
• Collaborate with Support Coordinators, families, caregivers, and the interdisciplinary team to ensure continuity of care.
• Provide caregiver training and home programs to reinforce therapeutic goals between sessions.
• Adhere to DDD policies, including HIPAA confidentiality standards and required laws.
• Maintain accurate billing records consistent with DDD rates and authorizations.
• Complete required DDD-mandated trainings.
