Job summary

Beyond Home Health Care, a NJ DDD-Approved provider, is seeking a licensed Speech-Language Pathologist for in-home and community-based services across New Jersey. The SLP will evaluate and treat language, articulation, social communication, AAC, and dysphagia per NJ DDD guidelines and person-centered planning.

The role offers per diem/contract engagement with flexible scheduling, built caseload by county, and supportive clinical leadership. Requires NJ licensure, ASHA CCC-SLP, and CPR/BLS.

Qualifications

• NJ license required to practice in New Jersey.
• ASHA CCC-SLP certification required; CFs considered with supervision.
• Master's degree in Speech-Language Pathology from an accredited program.
• Minimum of one year of clinical experience preferred; CFs and new grads considered.
• CPR/BLS certification required.
• Clean criminal background check and fingerprinting per NJ DDD.
• Reliable transportation and valid NJ driver’s license.

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans aligned with ISP and person-centered goals.
• Deliver one-on-one therapy in home and community settings.
• Document services per DDD guidelines, including session and progress notes, on time.
• Collaborate with Support Coordinators, families, caregivers, and the interdisciplinary team.
• Provide caregiver training and home programs to reinforce goals between sessions.
• Adhere to DDD policies, Danielle's Law, HIPAA, and related requirements.
• Maintain billing records consistent with DDD rates and authorizations.
• Complete required DDD trainings (Pre-Service, Preventing Abuse/Neglect/Exploitation, etc.).
