Job summary

Beyond Home Health Care is a NJ DDD-Approved Provider seeking a licensed Speech-Language Pathologist for in-home and community-based services across New Jersey. You will evaluate and treat language, articulation, AAC needs, and dysphagia in adults with developmental disabilities, following NJ DDD guidelines and person-centered planning.

The role emphasizes one-on-one therapy in home and community settings, collaboration with coordinating teams, caregiver training, and adherence to HIPAA and DDD

Qualifications

• NJ license as a Speech-Language Pathologist
• CCC-SLP required; CFs considered with supervision
• Master’s degree in Speech-Language Pathology from an accredited program
• Minimum of one year of clinical experience preferred; CFs/new grads considered
• Current CPR/BLS certification
• Clean criminal background check and fingerprinting per NJ DDD requirements
• Reliable transportation, valid NJ driver’s license, and current auto insurance
• Ability to pass DDD background checks including CRoff

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans aligned with ISP and person-centered goals
• Deliver one-on-one therapy in home and community settings
• Document services per DDD standards with session notes and progress notes
• Collaborate with Support Coordinators, families, caregivers and the team
• Provide caregiver training and home programs to reinforce goals
• Adhere to DDD policies including Danielle's Law, Stephen Komninos' Law, HIPAA
• Maintain accurate billing records per DDD Fee-for-Service rates
• Complete required DDD trainings (Pre-Service, abuse/neglect, etc.)
