Job summary

Beyond Home Health Care in New Jersey is seeking a licensed Speech-Language Pathologist to provide in-home and community-based services to adults with developmental disabilities. You will evaluate and treat language, speech, AAC needs, and dysphagia in accordance with NJ DDD guidelines.

The role emphasizes collaboration with supports coordinators and families, client-centered planning, and adherence to DDD policies, with flexible per diem scheduling and mileage considerations for visits.

Qualifications

• Current New Jersey license as a Speech-Language Pathologist issued by the NJ Audiology and Speech-Language Pathology Advisory Committee.
• ASHA Certificate of Clinical Competence (CCC-SLP) required; CFs considered with appropriate supervision.
• Master's degree in Speech-Language Pathology from an accredited program.

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans aligned with ISP and person-centered goals.
• Deliver one-on-one therapy services in the participant's home and community settings.
• Document services in compliance with DDD documentation standards, including session notes, progress notes, and outcome measures.
• Collaborate with Support Coordinators, families, caregivers, and the interdisciplinary team to ensure continuity of care.
• Provide caregiver training and home programs to reinforce therapeutic goals between sessions.
