Job summary

Beyond Home Health Care is seeking a licensed Speech-Language Pathologist to provide in-home and community-based speech, language, and swallowing services to adults with developmental disabilities in New Jersey. You will evaluate and treat communication and dysphagia per NJ DDD guidelines and person-centered planning.

Responsibilities include assessments, treatment planning, caregiver training, documentation, and collaboration with coordinators, families, and the interdisciplinary team.

Qualifications

• Current NJ license as an SLP issued by the NJ Audiology and Speech-Language Pathology Advisory Committee.
• ASHA CCC-SLP certification required; CFs considered with supervision.

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans aligned with ISP and person-centered goals.
• Deliver one-on-one therapy services in home and community settings.
• Document services per DDD standards, including session notes, progress notes, and outcome measures.
• Collaborate with Support Coordinators, families, caregivers, and the interdisciplinary team to ensure continuity of care.
• Provide caregiver training and home programs to reinforce therapeutic goals between sessions.
• Adhere to DDD policies, HIPAA confidentiality standards, and related laws.
• Maintain accurate billing records consistent with DDD Fee-for-Service rates and authorizations.
• Complete required DDD trainings (Pre-Service, Danielle's Law, Stephen Komninos' Law, etc.).
