Job summary

Optum, part of the UnitedHealth Group family, is seeking a Physical Therapist to deliver skilled services in patients' homes. The role collaborates with patients, caregivers, and an interdisciplinary team to promote mobility, restore function, and improve safety and independence within home settings.

You will reexamine patients at least every 30 days, tailor interventions, and participate in discharge planning, with additional duties as assigned.

Qualifications

• Current license to practice physical therapy in the assigned state.
• Graduate from a CAPTE-accredited physical therapy program.
• 1+ year of home health physical therapy experience recommended; mentoring possible.
• Current CPR certification is required.

Responsibilities

• Help patients maintain, restore, and improve movement and function.
• Provide interventions, perform reexaminations every 30 days, adjust plans, and discharge planning.
• Perform other duties as assigned.
