Job summary

UnitedHealth Group's UP Home Health, part of the Optum family, is seeking a Physical Therapist for Home Health to assess functional status, mobility, endurance, and pain, and to develop and execute a care plan in patients' homes. You will evaluate needs, guide treatment, and monitor progress while collaborating with physicians and nurses.

Responsibilities include documenting visits, communicating progress to patients and families, and ensuring services comply with state guidelines.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to dependable vehicle.
• CPR certification preferred or ability to complete within 90 days of hire.

Responsibilities

• Evaluates patient’s functional status and PT needs as part of care plan development.
• Provides PT services within scope of practice and plan of care.
• Observes and reports patient responses to treatment to nurse/physician.
• Treats patients to relieve pain, restore function, and maximize independence.
• Instructs patient and family on plan of care, limitations, and progress.
• Visits patients per Plan of Treatment and documents progress notes.
• Completes evaluations and develops PT plan within state guidelines.
• Reports outcomes and anticipated frequency of care.
• Demonstrates sensitivity to diverse patient backgrounds.
