Qualifications

• Current Physical Therapist license in the state of practice.

Responsibilities

• Evaluate patient functional status and PT needs.
• Assist physician in developing the plan of care.
• Provide PT services within scope of practice and plan of care.
• Observe and report patient responses to treatment to supervisor/physician.
• Treat patients to relieve pain, restore function and maintain maximum functioning.
• Instruct patients and family on plan of care and progress.
• Complete evaluations and develop PT plan of care per state guidelines.
• Document outcomes of evaluation, goals, and projected frequency of care.
