Job summary

Amedisys, part of the Optum family, is seeking a Physical Therapist to deliver skilled therapy in patients' homes, promoting mobility and independence. You will collaborate with patients, caregivers and an interdisciplinary team to support recovery and prevent hospitalizations.

Responsibilities include reexaminations every 30 days, adjusting interventions as needed, and discharge planning, with a focus on safety and quality of life within the home setting. A valid PT license and CPR are required.

Qualifications

• Graduate from an CAPTE-accredited physical therapy program.
• Current state license to practice PT.
• Current CPR certification.
• 1+ year of PT experience in home health (or approval by regional leaders for less than 1 year).
• Driver’s license and reliable transportation.

Responsibilities

• Provide skilled physical therapy in patients' homes to promote mobility and safety.
• Perform interventions, reexaminations (at least every 30 days), adjust plans as needed, and discharge planning.
• Collaborate with patients, caregivers and care team to support recovery.
