Job summary

UnitedHealth Group is seeking a licensed Physical Therapist to provide skilled PT services in patients' homes, promoting mobility, independence and safety. You’ll collaborate with patients, caregivers and an interdisciplinary team to support recovery and prevent hospitalizations.

Requirements include a CAPTE-accredited PT program, active state license, CPR certification and a driving license with reliable transportation.

Qualifications

• Current license to practice physical therapy in the state you’re assigned.
• Graduate from an accredited physical therapy educational program as set by CAPTE.
• 1+ related physical therapy experience for PTs entering home health care.
• If less than one year of experience, approval from regional clinical leaders and mentoring/residency may be required.
• Current CPR certification.
• Driver’s License and access to reliable transportation.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Provide interventions, conduct reexaminations (every 30 days minimum), modify interventions and perform discharge planning.
• Other duties as assigned.
