Job summary

Primary responsibilities include helping patients improve movement, conducting reexaminations every 30 days, modifying plans as needed, and discharge planning.

Qualifications

• Graduate from CAPTE-accredited physical therapy program.
• Current license to practice physical therapy in the assigned state.
• Driver’s license and reliable transportation.
• Current CPR certification.
• 1+ years of home health PT experience or approval/mentoring if less than 1 year.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Provide interventions, conduct reexaminations every 30 days, modify interventions as necessary, and perform discharge planning.
• Collaborate with patients, caregivers and interdisciplinary care team to support recovery and improve quality of life in the home setting.
