Job summary

Qualifications

• Graduate from an CAPTE-accredited physical therapy program.
• Current license to practice physical therapy in the state of the assignment.
• Current CPR certification.
• 1+ years of physical therapy experience in home health care or approval/mentoring if less than 1 year.

Responsibilities

• Provide skilled physical therapy services to patients in the home setting to promote mobility and function.
• Perform reassessments at least every 30 days and adjust interventions; conduct discharge planning.
• Collaborate with patients, caregivers, and interdisciplinary team to support recovery and safety.
