Job summary

Optum in the UnitedHealth Group family seeks a dedicated Physical Therapist to join our Home Health team. You will assess patient needs, determine a treatment plan, and provide interventions to improve mobility and well-being in home settings.

Responsibilities include evaluating function, coordinating with physicians and nurses, documenting progress, and educating patients and families while delivering compassionate care in diverse home environments.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance, access to a dependable vehicle or public transportation.
• Ability to function in any home situation regardless of client characteristics.
• Preferred: CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs; assists the physician in plan of care.
• Provides services within the scope of practice and in coordination with the health care team.
• Observes and reports patient response to treatment and any changes in condition.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs patients and family/caregivers regarding plan of care and progress.
• Visits patients per the Plan of Treatment and submits progress notes per policy.
• Completes evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes, goals, and anticipated frequency of care.
