Job summary

Optum, a member of the UnitedHealth Group family, is seeking a Physical Therapist to deliver skilled services in patients' homes in the Oak Ridge area. You will promote mobility, restore function, and support safety and independence, working with patients, caregivers, and a multidisciplinary team to enhance quality of life.

The role requires CAPTE-accredited PT education, an active state license, CPR certification, a valid driver’s license, and at least 1 year of PT experience in home health

Qualifications

• Graduate from CAPTE accredited physical therapy program.
• Current license to practice physical therapy in the state of assignment.
• Driver's license and access to reliable transportation.
• Current CPR certification.
• 1+ years related PT experience in home health care; if less than 1 year, regional clinical leaders' approval and mentoring/residency required.

Responsibilities

• Provide skilled physical therapy services in patients' homes to promote mobility and independence.
• Perform reexaminations every 30 days at minimum, modify interventions as needed, and conduct discharge planning.
• Collaborate with patients, caregivers, and interdisciplinary care team to support recovery and prevent hospitalizations.
• Other duties as assigned.
