Job summary

Powerback Rehabilitation To You is seeking a licensed Physical Therapist Assistant to deliver outpatient rehab in home and in independent living settings across the region. You will support patients with functional recovery and participate in a patient-centered care program.

Under the supervision of a licensed PT, you will assess needs, implement treatment plans, document care, educate families, and help patients regain independence. A flexible schedule and nationwide travel may be involved.

Qualifications

• Must have a valid license in the state(s) of practice.
• Must have a valid driver's license or reliable transportation to and from patients' residence.

Responsibilities

• Provides outpatient rehabilitation in home/in-home settings under supervision of a Licensed Physical Therapist.
• Evaluates and treats patients to facilitate independence and functioning in their home or facility setting.
• Maintains timely and accurate documentation for all patients treated and for billing purposes.
• Educates patients, families and caregivers on skills and techniques of the physical therapy treatment program.
• Maintains equipment and reports needs for repair; adheres to treatment schedules and team plans.
• Implements individualized treatment plans and participates in interdisciplinary care conferences.
