Job summary

Powerback is seeking a Physical Therapist Assistant to deliver outpatient rehabilitation services in in-home settings for older adults. The role supports functional independence, under supervision of a Licensed Physical Therapist, with a flexible schedule across various settings including home visits.

The position emphasizes patient-centered care, collaboration with the rehab team, and adherence to treatment plans while maintaining documentation and equipment needs.

Qualifications

• Must have a valid license in the state(s) of practice, or proof of license eligibility.
• Must have a valid driver's license or reliable transportation to and from patients' residence.

Responsibilities

• Attends and contributes to patient care, staffing conferences and other required meetings as a representative of physical therapy, under the direction of the physical therapist.
• Adheres to department and patient treatment schedules, and completes timely clinical and billing documentation.
• Collaborates with rehabilitation team to implement programs matching patient needs.
• Assists the Physical Therapist during tests, evaluations, and complex procedures.
• Instructs patients and families in therapy techniques under supervision.
• Maintains timely documentation for all patients treated.
• Cleans and maintains equipment and reports repair needs.
• Prioritizes patient service in a compassionate setting and follows the individualized treatment plan.
