Powerback Rehabilitation To You is seeking a licensed Physical Therapist to provide outpatient rehab in patients' homes across the United States. The role emphasizes individualized programs to restore function and promote independence in older adults, with flexibility to tailor care to each home environment.

Formación

• Must have a valid Physical Therapy license in the state(s) of practice.
• Master's degree in Physical Therapy; or Master's in related field plus Bachelor's in PT; or Bachelor's plus 5 years PT experience.
• The qualifying degree earned outside the U.S. must be evaluated to be the U.S. equivalent.
• Must have a valid driver's license or reliable transportation.

Responsabilidades

• Provide outpatient rehabilitation services in the patient's home and in home-based settings.
• Assess, develop, and deliver physical therapy plans to restore function and independence.
• Collaborate with the rehabilitation team and refer to other disciplines as needed.
• Supervise PTAs and students per licensure requirements.
• Maintain timely documentation and billing records.
• Ensure safety and adjust plans to patient needs.
