Job summary

Powerback is seeking a Physical Therapist to deliver in-home outpatient rehabilitation for older adults. You will assess patients, design individualized plans, and guide families to maximize independence in home settings.

The role emphasizes collaboration with a multi-disciplinary team, supervision of assistants, and adherence to documentation standards while providing compassionate, high-quality care in diverse environments.

Qualifications

• Must have a valid Physical Therapy license in the state of practice.
• Master's degree in Physical Therapy or equivalent.
• Proven ability to work independently in home- and community-based settings.

Responsibilities

• Provide outpatient rehabilitation services in a home setting for older adults.
• Develop and deliver individualized PT plans and goals.
• Supervise assistants and ensure timely documentation and meetings.
• Educate patients and caregivers on home exercise, safety, and independence.
• Collaborate with the rehab team for comprehensive care.
• Maintain equipment, order supplies, and ensure patient safety.
