Job summary

Powerback Rehabilitation is seeking a licensed Physical Therapist to provide in-home outpatient rehabilitation for older adults across various settings. You will evaluate, design, and implement individualized PT programs, educate patients and caregivers, and collaborate with the rehab team to optimize functional outcomes.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility.
• Master’s degree in Physical Therapy (or PT-related master’s with Bachelor’s in PT).
• Must have a valid driver’s license or reliable transportation to patients’ residences.

Responsibilities

• Provide outpatient rehabilitation services in-home for older adults.
• Assess, develop, and deliver PT programs to facilitate rehabilitation.
• Educate patients and caregivers to promote independence and safety.
• Coordinate with rehabilitation team and refer to other disciplines as needed.
• Maintain documentation and schedule adherence.
• Order supplies and equipment as necessary.
• Promote a safe home environment and correct hazards.
• Supervise PT Assistants and students as required.
