Job summary

Powerback Rehabilitation To You seeks a dedicated Physical Therapist to deliver outpatient rehabilitation services in patients’ homes. You will evaluate needs, design tailored therapy plans, and work to restore function in older adults across home and community settings.

The role requires a valid PT license in the relevant state(s), a graduate degree in PT, and reliable transportation to visit patients’ residences. A supportive team and flexible scheduling will be provided.

Qualifications

• Valid Physical Therapy license in the state(s) of practice.
• Master's degree in Physical Therapy or equivalent in PT-related field with Bachelor's in PT.
• Valid driver's license or reliable transportation to/from patients' residences.

Responsibilities

• Provide outpatient rehabilitation services in in-home setting for older adults.
• Assess needs, develop and deliver PT programs to facilitate rehabilitation.
• Refer to other disciplines as needed to address comprehensive patient needs.
• Educate patients and caregivers to promote independence and functional goals.
• Supervise PT Assistants and other staff as required by licensure.
