Job summary

Powerback Rehabilitation To You seeks a dedicated Physical Therapist to provide in-home outpatient rehab for older adults. You will assess, plan, and implement individualized therapy programs in patients’ homes or in independent living settings.

The role requires a valid PT license, a Master’s degree in Physical Therapy, and US driving access. Experience in home health is preferred; travel across the service area may be required. Competitive benefits are offered.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice.
• Master's degree in Physical Therapy (or equivalent per state).
• Driver's license or reliable transportation.
• Experience providing in-home/outpatient rehabilitation is preferred.

Responsibilities

• Provide outpatient rehabilitation services in an in-home setting for older adults.
• Evaluate, develop, and deliver individualized physical therapy programs in the patient’s home or in an independent living facility.
• Supervise PT assistants or students as applicable and ensure adherence to licensure standards.
• Maintain timely documentation and align with treatment schedules and meetings.
• Collaborate with the rehabilitation team to optimize patient care and referrals to other disciplines.
• Educate patients and caregivers on exercises, safety, and home programs.
