Qualifications

Responsibilities

• Provide outpatient rehabilitation in in-home settings for older adults.
• Develop and deliver individualized physical therapy programs.
• Educate patients and caregivers on exercises and safety.
• Collaborate with the rehab team to optimize care plans.
• Maintain thorough documentation and billing records.
• Coordinate referrals to other disciplines as needed.
• Ensure a safe environment and proper equipment handling.
