Job summary

Powerback Rehabilitation To You is seeking a dedicated Physical Therapist for full-time, outpatient/home-based rehab. You will evaluate and treat older adults in homes or independent living facilities, delivering individualized therapy plans to improve function and independence.

You'll educate patients and caregivers, coordinate with the rehab team, and ensure high-quality documentation. Mentorship for new grads, free CEUs, and visa sponsorship are available.

Qualifications

• Must hold a Physical Therapy license in the state of practice or eligibility.
• Graduate degree(s) in Physical Therapy as listed in qualifications.
• Commitment to ongoing professional development and evidence-based practice.

Responsibilities

• Provide outpatient rehabilitation services in-home or in living facilities for older adults.
• Develop and deliver individualized PT plans; assess progress and adjust as needed.
• Educate patients and caregivers; promote independence and safe mobility.
• Collaborate with rehab team; refer to other disciplines as appropriate.
• Supervise PT assistants and trainees; ensure schedule adherence and proper documentation.
• Participate in meetings, care conferences, and program development activities.
• Support safety, equipment management, and home modification recommendations.
