Job summary

BrightStar Care of Concord, Lexington, and Woburn is seeking a licensed Physical Therapist for part-time / per diem home health visits. You will assess clients in their homes, design personalized plans, and deliver interventions to improve strength, balance, mobility, and safety.

Candidates must hold an MA PT license and DPT or MPT; CPR certification is preferred. Expect mileage reimbursement and a flexible schedule with local visits and a supportive team.

Qualifications

• Active MA PT license required.
• DPT or MPT from an accredited program.
• CPR certification preferred.

Responsibilities

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to improve strength, balance, mobility and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
• Document care using a mobile-friendly EMR.
