Job summary

BrightStar Care – Concord, Lexington & Woburn is seeking a licensed Physical Therapist to join our home health team on a part-time/per diem basis. You will assess patients in their homes, develop personalized plans, and deliver skilled interventions to improve strength, balance, mobility, and safety.

Travel to local visits is required and documentation is managed via a mobile EMR. Eligible candidates have MA PT licensure (DPT or MPT preferred), CPR certification, and at least 1 year of

Responsibilities

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to improve strength, balance, mobility, and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
• Document care using mobile EMR.
