Job summary

BrightStar Care of Concord, Lexington and Woburn is seeking a licensed Physical Therapist to provide in-home PT services part-time or per diem. You will evaluate patients, design personalized plans, and deliver interventions to improve strength, balance, mobility and safety in home settings.

Requirements include MA license, DPT/MPT, CPR, and a valid driver's license with reliable transportation; new grads welcome. Mileage reimbursement offered and flexible schedule available.

Qualifications

• Active MA PT license required.
• DPT or MPT from an accredited program is required.
• CPR certification is required.
• 1+ year of home health PT experience is preferred; new grads welcome.

Responsibilities

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to improve strength, balance, mobility and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with an interdisciplinary care team.
• Document care using a mobile EMR system.

Skills

Active MA PT license

CPR certified

1+ year experience preferred

Reliable vehicle & driver's license
