Job summary

BrightStar Care – Concord, Lexington & Woburn is seeking a licensed Physical Therapist to provide in-home physical therapy services on a part-time or per diem basis. You will evaluate patients, design personalized care plans, and help improve strength, balance, mobility, and safety in their home environments.

Ideal candidates hold an MA PT license, a DPT or MPT, CPR certification, and have 1+ year of experience (new grads welcome). A reliable vehicle and driver's license are required.

Qualifications

• DPT or MPT from an accredited program
• CPR certification
• 1+ year of experience preferred (new grads welcome)
• Reliable vehicle & license

Responsibilities

• Perform in-home PT evaluations and create personalized care plans
• Provide skilled PT interventions to enhance strength, balance, mobility, and safety
• Educate clients and families on exercises and home safety
• Collaborate with a multidisciplinary care team
• Document care using mobile-friendly EMR
