Job summary

BrightStar Care – Concord, Lexington & Woburn is seeking a licensed Physical Therapist (PT) to work in-home on a part-time/per diem basis. You will assess patients, develop personalized plans, and provide interventions to improve strength, balance, mobility and safety in the clients home.

Requirements include an active MA PT license, DPT or MPT, CPR certification, and a reliable vehicle. Flexible scheduling and mileage reimbursement are offered to support home visits.

Qualifications

• Active MA PT license
• CPR certified
• 1+ year of experience preferred
• Reliable vehicle and valid drivers license

Responsibilities

• Perform in-home PT evaluations and create personalized care plans
• Provide skilled PT interventions to enhance strength, balance, mobility, and safety
• Educate clients and families on exercises and home safety
• Collaborate with a multidisciplinary care team
• Document care using mobile-friendly EMR
