Job summary

BrightStar Care in Concord, Lexington & Woburn seeks a licensed Physical Therapist (PT) for part-time / per diem home visits. You will evaluate clients at home, design personalized plans and deliver interventions to improve strength, balance and safety.

Requirements include MA PT license, DPT or MPT, CPR certification, and 1+ year experience preferred. A reliable vehicle and valid license are required; mileage reimbursement is provided.

Qualifications

• Active MA PT license required.
• DPT or MPT from an accredited program.
• CPR certification required.

Responsibilities

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to enhance strength, balance, mobility, and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
• Document care using mobile-friendly EMR.
