Job summary

BrightStar Care of Concord, Lexington and Woburn is seeking a licensed Physical Therapist to join our in-home care team on a part-time or per diem basis. You will evaluate patients in their homes and develop personalized plans to improve strength, balance, mobility, and safety.

We offer flexible scheduling, mileage reimbursement, and a mobile EMR for streamlined documentation. Active MA PT license and DPT/MPT are required, with CPR certification and at least 1 year of experience preferred.

Qualifications

• Active MA PT license required.
• Graduate degree in Physical Therapy (DPT or MPT) from an accredited program.
• CPR certification is required.

Responsibilities

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to enhance strength, balance, mobility, and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
• Document care using mobile-friendly EMR.
