Job summary

BrightStar Care – Concord, Lexington & Woburn is seeking a licensed Physical Therapist (PT) to provide in-home therapy services on a part-time / per diem basis. You will evaluate patients at home, design personalized care plans, and deliver interventions to improve strength, balance, mobility, and safety, with documentation via a mobile EMR.

Flexible scheduling, mileage reimbursement, and a supportive team are offered; MA license, DPT/MPT, CPR, and reliable vehicle are required.

Qualifications

• Active MA PT license.
• DPT or MPT from an accredited program.
• CPR certified.
• 1+ year of experience preferred (new grads welcome).
• Reliable vehicle & valid driver’s license.

Responsibilities

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to enhance strength, balance, mobility, and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
• Document care using mobile-friendly EMR.
