Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice.
• Master's degree in Physical Therapy or equivalent.
• Bachelor's degree in Physical Therapy plus 5 years PT experience.
• Ability to work in home-based and community settings.

Responsibilities

• Provide outpatient rehabilitation in home and community settings.
• Supervise PTAs, temporary licensees, students as allowed.
• Document care timely and participate in team meetings.
• Assess patient needs, create and modify treatment plans.
• Educate patients and caregivers to promote independence.
