Job summary

Powerback Rehabilitation is seeking a part-time Physical Therapist to provide in-home outpatient rehabilitation for older adults across Agawam, MA and nearby areas. The role supports a flexible schedule (about 20–25 hours per week) and delivers personalized therapy in patients' homes or in independent living settings.

Qualifications

• Valid Physical Therapy license in the state(s) of practice.
• Master's degree in Physical Therapy or related field with PT degree.
• US degree evaluation if earned outside the U.S.
• Driver's license or reliable transportation to patients' residences.
• Experience in home health or outpatient rehab is preferred.

Responsibilities

• Supervise PT assistants and students per state licensure requirements.
• Adhere to treatment schedules and complete documentation timely.
• Join patient care and staffing conferences and related meetings.
• Refer to other disciplines to address patient needs.
• Collaborate with rehab team to tailor therapy plans.
• Adapt plans to individual patient needs and safety.
• Ensure a safe environment and equipment maintenance.
• Order supplies and manage equipment as needed.
• Educate patients and caregivers for independence.
• Perform detailed PT evaluations and progress documentation.
• Deliver home-based or facility-based therapy as appropriate.
• Integrate health and wellness programming into care plans.
• Assist with home and community mobility assessments.
• Perform additional related duties as required.
