Job summary

Powerback is seeking a Physical Therapist to deliver outpatient rehabilitation in home-based settings across Westfield, MA and beyond. The role emphasizes patient-centered care, functional restoration, and flexibility to work in patients’ homes or affiliated facilities.

Qualified candidates will hold a PT license in the state and have a Master’s in Physical Therapy, with potential for part-time to full-time hours.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice
• Master's degree in Physical Therapy; or Master’s in a PT-related field plus a Bachelor's in PT
• Ability to work in home-based/outpatient settings and coordinate care with caregivers

Responsibilities

• Provide outpatient rehabilitation services in home or facility settings
• Develop and deliver individualized PT programs to restore and improve function
• Educate patients and caregivers on skills for independence and safe mobility
• Coordinate with rehab team to address comprehensive patient needs
• Maintain documentation and ensure timely billing and scheduling requirements
