Qualifications

• Must have a valid Physical Therapy license in the state of practice.
• Master's degree in Physical Therapy or Bachelor's degree with 5 years of PT experience.
• Foreign PT degree must be evaluated to US equivalence if earned outside the US.

Responsibilities

• Provide outpatient rehabilitation services in the home or other patient environments.
• Develop and deliver individualized PT programs to restore function.
• Educate patients and caregivers to promote independence and safe home mobility.
• Supervise PT assistants, temporary licensees, students and support personnel as required.
• Maintain timely documentation, scheduling, and referrals to address patient needs.

Education

Master's degree in Physical Therapy
