Job summary

Optum, through Almost Family and LHC Group, delivers home-based care nationwide. We are hiring a Per Diem Personal Care Assistant to support individuals at home, assisting with daily living activities and personal care.

Responsibilities include bathing, grooming, meal preparation, mobility assistance, and transportation to appointments as needed. Requires reliability, a valid driver’s license, and the ability to work flexible hours; CPR certification preferred.

Qualifications

• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification preferred or able to obtain within 90 days of hire.
• 6+ months of home care experience.
• Ability to work flexible hours as required to meet clients’ needs.

Responsibilities

• Assist with activities of daily living (bathing, grooming, denture care, nail care, dressing).
• Provide or assist with medication reminders and basic health monitoring.
• Assist with transfers and ambulation with cane, walker, or crutches.
• Prepare meals and assist with feeding as needed.
• Keep client home safe with proper use of equipment (wheelchairs, lifts).
• Accompany client to medical appointments or errands when required.
