Job summary

Optum, via Access Personal Care Services, is seeking Per Diem Personal Care Assistants I or II to provide in-home support and environmental, nutritional, personal care, and household assistance to help clients live independently at home.

Responsibilities include assisting with activities of daily living, meals, mobility, and transportation to appointments, while documenting care and reporting changes to the supervisor. CPR preferred; flexible hours and driving to client homes expected.

Qualifications

• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• 6+ months of home care experience.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Provide or assist with activities of daily living (bathing, hair care, denture care, nail care, skin care, transfers, ambulation, dressing, medication reminders).
• Live-in and sleep-over support including overnight presence when required in applicable regions.
• Utilize Electronic Visit Verification (EVV) as applicable and assist with essential household tasks.
• Accompany clients to medical appointments or shopping and assist with errands as needed.
• Record vital signs (temperature, pulse, respiration, blood pressure) when ordered within training limits.
• Prepare meals according to dietary needs and assist with feeding if required.
• Maintain a clean, safe home environment and safe use of equipment such as rails, oxygen, wheelchairs.
• Record and report observations to Branch Manager and notify immediately of incidents.
