Job summary

CenterWell Home Health is seeking an Occupational Therapist to deliver patient-centered rehab services in home settings, focusing on improving strength, mobility, balance, and daily functioning. You will work with physicians and a multidisciplinary team to implement and adjust the Plan of Care.

The role requires OT licensure, CPR certification, and a valid driver’s license with reliable transportation. Home health experience is preferred; six months of OT practice recommended.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.
• Minimum six months of occupational therapy experience.
• Home Health experience.

Responsibilities

• Assess/screen patient's daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
• Administer and interpret diagnostic and prognostic tests of function to patients in their homes or other settings.
• Confer with physician and care team to develop/revise the Plan of Care.
• Provide therapeutic treatment to improve/restore strength, coordination, ROM and function.
• Review and expand retraining programs aligned with patient goals and rehab guidelines.
• Consult with vocational team members to plan patient-centric outcomes.
• Report patient status to physician and care management staff.
• Participate in care coordination and discharge planning and serve as rehab resource.
