Job summary

CenterWell Home Health seeks a licensed Occupational Therapist to deliver home-based rehabilitation, focusing on restoring independence and function for patients in their homes. You will collaborate with physicians, nurses, and aides to tailor therapy plans that improve strength, mobility and daily living skills.

Ideal candidates have an active OT license, CPR certification, and reliable transportation to visit patients' homes as part of a full-time schedule.

Qualifications

• OT licensure required.
• CPR certification required.
• Valid driver's license with auto insurance and reliable transportation required.

Responsibilities

• Assess/screen daily living/work-related skills and develop retraining programs with measurable objectives.
• Confer with physician and care team on Plan of Care revisions.
• Provide therapeutic treatment per physician orders to improve strength, ROM and function.
• Document patient status, progress and discharge summaries accurately.
• Design community reintegration and adaptive equipment plans with interdisciplinary team.
