CenterWell Home Health is hiring an Occupational Therapist to provide in-home rehabilitation focused on restoring function and independence. You will evaluate daily living skills, collaborate with physicians and the care team, and implement therapy plans in patients' homes to improve strength, mobility and everyday activities.

We require an active OT license, CPR certification, and a valid driver's license with reliable transportation.

Qualificações

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.
• Minimum six months of occupational therapy experience.
• Home Health experience.

Responsabilidades

• Assess/screen patient's daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
• Confer with the patient's physician and health care team and participate in the Plan of Care.
• Provide therapeutic treatment and instruction to improve/restore strength, coordination and range of motion.
• Review/expand retraining programs to meet individual behavioral goals within the rehab program.
