Job summary

CenterWell Home Health is hiring an occupational therapist to help patients regain strength, mobility and independence in their homes. You will work with physicians and the rehab team to implement plans of care and to provide hands-on therapy focused on outcomes.

The role requires current OT licensure, CPR certification, and a valid driver’s license with reliable transportation. Travel to patient homes is expected as part of a patient-centered home health program.

Qualifications

• Current and unrestricted OT licensure is required.
• Current CPR certification is required.
• Valid driver’s license with auto insurance and reliable transportation required.

Responsibilities

• Assess/screen patients' daily living and work-related skills and develop retraining programs.
• Administer and interpret function tests to assess level of function.
• Collaborate with physicians and rehab team to update the care plan.
• Deliver therapeutic treatment to improve strength, coordination, ROM and function.
• Document patient status, progress and care plans accurately.
