Job summary

CenterWell Home Health is seeking a licensed Occupational Therapist to deliver in-home rehabilitation, focusing on restoring function and independence. You will work with physicians and the rehab team to assess needs, plan, implement and document therapeutic interventions that improve strength, mobility and daily living skills.

The role involves driving to patient homes, coordinating discharge planning, and mentoring staff while upholding high-quality patient care standards.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.

Responsibilities

• Assess/screen patients' daily living/work-related skills and create retraining programs.
• Collaborate with physicians and rehab team on care plans.
• Provide therapy to improve strength, ROM, coordination and function.
• Update retraining programs to align with patient goals.
• Coordinate with vocational team for patient-specific plans.
• Report patient status and progress to care team and family.
• Participate in discharge planning and care coordination.
• Design community reintegration activities and adapt equipment.
• Train staff and family to perform retraining tasks.
• Supervise OT Assistant and Home Health Aide.
• Document care observations, interventions and plans accurately.
