Job summary

You will work with physicians, nurses and therapists to develop and execute individualized care plans and document patient progress daily.

The role requires current OT licensure, CPR certification and reliable transportation to patient sites; previous home health experience is a plus.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.

Responsibilities

• Assess daily living/work-related skills and develop retraining programs.
• Confer with physician and care team; participate in Plan of Care updates.
• Provide therapeutic treatment to improve strength, coordination and function.
• Document patient progress and care observations in day services records.
• Coordinate care and participate in discharge planning with the team.
